Wednesday, May 22nd- 8-3 p.m.
Thursday, May 23rd - 1-7 p.m.
If you live in the Litchfield School District and have a child
who will be 3 years old on or before 9/1/2019,
please give Mrs. Bishop a call @ 324-3514 to schedule a screening appointment.
2019-2020 School Year
You will need to provide the following information for your child’s records. This information is mandated by the State of Illinois. Please provide this information to the Pre-K office on or before the first day of school.
Current Illinois Physical
Health history information completed and signed (on the physical form)
Certified Birth Certificate
Immunizations record
Lead Screening date and result. Any earlier one is sufficient.
