The following information is mandated by the State of Illinois.
Please provide the following information to the Pre-K office on or before the first day of school.
Current Illinois Physical
Health history information completed and signed (on the physical form)
Certified Birth Certificate
Immunizations record
Lead Screening date and result. Any earlier one is sufficient.
Income Verification –This is a new mandated requirement that we have to have in your child’s file.
Only one bullet point is needed.
Pay stubs-2 most recent, consecutive
· Proof of WiC benefit
· Proof of Snap
· Proof of TAnf
· Proof of SSI
· Proof of CCAP
· Tax return-most recent
· Wages and tax statement-most recent W2
· Verification letter from employer
· Proof that parent is enrolled in Medicaid-not children's medical card
· Signed written statement from the family provide form for families with no income
Children entering Pre-School will have to provide the necessary physical and immunization documentation on the first day of school in order to attend class.
